This user guide is for Supervisors who want to incorporate the self-review in the final University Staff employees who use Cornerstone to complete their Final Evaluation. This Final Evaluation includes an option to complete a self-evaluation. This guide will follow the steps to complete the University Final Evaluation task if your department/team does participate in self-reviews.
The order of the task is as following: Supervisor → Employee → Supervisor → Employee

Navigate to Cornerstone
Scroll down to the task box and select “20xx-20xx University Staff Final Evaluation”
Note: if you have more than 10 tasks, you must clock select the “Task Box (Action Required)” to view all the tasks
Navigate to the left hand side and select the employee you would like to begin the final evaluation for
On the first page where it reads“Supervisor Acknowledgment of Self-Evaluation”Section 1 Directions, thoroughly read the instructions. The graphic will highlight the current section in Gold.
Select Nextto Navigate to Supervisor Yes/No Selection for Employee Self-Evaluation
Select YES or NO from the dropdown .

Once the supervisor has selected YES, click Next and navigate to the Employee Goal Section & Self Review (If Applicable).
Once at the Goal Section page, scroll down to the end of the page and click Submit. You will see a pop-up asking if you are sure; click Submit.
Notedepending on whether or not you would like the employee to complete a self-evaluation.
Once selected YES or NO from the dropdown, select Nextto navigate to the Employee’s Performance Plan
On the Employee’s Performance Plan, you may add comments on each goal if you prefer or scroll to the end and select Submitif you would not like to add comments. All comments will be viewable by the employee and sent to their personnel file.
NOTE: the supervisor will have the opportunity to add supervisor comments and ratings in Step Section 3: Supervisor Final Departmental Rating--clicking selecting Submit on this step will not fully submit the entire review.
At any time, the carrot next to the goals can be clicked and you will be able to view Comments made on the Performance Plan throughout the year, as well an expanded view of the goals. Note: the full goal and description is only viewable in the task by clicking the carrot next to the goal and clicking Manage Performance Plan
The supervisor will click Select Submit, and an additional Pop-Up will ask the Supervisor to click select submit again. Please ensure you are clicking submit selecting Submit twice to ensure the task is routing to the next step. This will be standard for every step.

Section 2: Employee Self-

Navigate to Cornerstone
Scroll down to the task box and select “Your 20xx-20xx University Staff Final Evaluation”
Note: if you have more than 10 tasks, you must clock select the “Task Box (Action Required)” to view all the tasks
Click Next and the employee Select Next. You will be navigated to the Section 2 Directions. Thoroughly read through these directions before continuing. The graphic will highlight the current section in Gold. Select Next to continue.
This section is the Supervisor Acknowledgement of Yes/No Selection for Employee Self-Evaluation Step. When you scroll down . In this section you will view whether or not your Supervisor would like you to complete a self-evaluation. Scroll down to the Yes/No Dropdown should read YES. This indicates that your supervisor would like you to use the following sections as Selection.
If the selection reads YES, use the next section to add comments to your goals in the form of a self-evaluation.

Click Next and the employee will be navigated to the Employee Goal Section & Self Review (If Applicable).

selection reads NO, you may skip through this section by selecting Next. Any comments you provide will be optional.
Select Next to navigate to the Employee’s Performance Plan.
You may now add comments under their goals as a form of a self-evaluation. Please add relevant comments that the supervisor can read and take into account when determining your goal ratings and final departmental rating. SelectNext when you are finished. If you are not using this section as a self-evaluation, please select Nextto navigate forward.
At any time, the carrot next to the goals can be clicked and you will be able to view Comments made on the Performance Plan throughout the year.
After the employee has finished adding comments to their goals, click Next
The next page is the Employee Employee’s Self-Review Evaluation Narrative (If Applicable). Here the employee can add a you may add an optional self-review evaluation narrative they you would like their the supervisor to view and consider when determining their goal ratings and a final departmental scorerating.
Click Select Submitonce at the bottom, and Submit again when the pop-up shows up once the employee has finished looking through the sections. The task will now be routed back to the Supervisor to complete the Supervisor Final EvaluationDepartmental Rating.

Navigate to Cornerstone
Scroll down to the task box and select “20xx-20xx University Staff Final Evaluation”
Note: if you have more than 10 tasks, you must click the “Task Box (Action Required)” to view all the tasks
Find Hover over the left hand and select the next step in the sequence, which reads “Supervisor Final Departmental Rating and Sign-Off”bar to ensure you are on the correct employee and the correct step. Select Next. You will be navigated to the Section 3 Directions. Thoroughly read through these directions before continuing. The graphic will highlight the current section in Gold. Select Next to continue.
Note, if you have multiple employees who are on different stages of the final evaluation, you must navigate to the left hand side and click the correct section. The green check mark next to your employee indicates that the step you are currently on has been completed. A blue circle next to the employee will indicate that it is a newly opened step and has not been completed.
The first section will be the Employee Employee’s Self-Review Evaluation Narrative (If Applicable). Here you can may view the employee’s self-review evaluation narrative and consider it when providing your final rating.
Click Next and the supervisor will be routed back to the Employee Goal Section. Now the supervisor has Select Next to move forward to the Employee’s Performance Plan. Here you have a chance to rate, weight, and comment on all the goals for the employee in the Employee's Performance Plan in the lens of a Final Evaluation. The Employee’s self-evaluation comments under each goal, if applicable, will be visible here for consideration.
At any time, the carrot next to the goals can be clicked and you will be able to view Comments made on the Performance Plan throughout the year.
After the supervisor has weighted and rated all the goals for the year, click NextThe supervisor ratings, weights, and comments are complete, select Next
You will be navigated to the Final Departmental Rating and Supervisor Narrative section where they you will determine a final rating for their the employee and add an optional narrative they you would like their the employee to see. All of this information will be saved in the employee’s personnel file.
Click Select Next and the supervisor will now be at the to navigate to the Supervisor Final Sign-Off page. Please read the instructions thoroughly. Click Save for Laterif the department is participating in a calibration meeting and the final rating must be approved. Sign the your name and click select Submit once at the bottom, and Submitagain when the pop-up shows up, and now the step is complete.
Now the The task will be sent to the employee where they will finalize the evaluation.

Click Next and the employee can view their Self-Review Narrative.
Click Next and the employee will be on the Employee Goal Section where they can view the ratings that their supervisor has provided them on each individual goal
Click Next and the employee Select Next. You will be navigated to the Section 4 Directions. Thoroughly read through these directions before continuing. The graphic will highlight the current section in Gold. Select Next to continue.
Select Next to view Employee’s Self-Evaluation Narrative.
Select Next to view the Employee’s Performance Plan. Here you can view the ratings and comments that the supervisor has provided you for each individual goal.
Select Next. You will be navigated to the Final Departmental Rating and Supervisor Narrative where they you can view their the supervisor’s final rating and add any comments they you would like to be stored in their your personnel file.
After the employee feels satisfied with their you have sufficiently reviewed the final review, they can click select Next where they you will have a chance to Acknowledge that you received a score by signing off.
Note you may Decline to Sign, but it will still indicate that you have received a score.
Click Select Submitand Submit again when the pop-up shows up and the review is complete!
Summary
At the end of the task for both employees and supervisors, a summary of the scores will be visible. This will show the average weighted score of the goals as well as the final departmental rating. This summary is for informational purposes and will be there as additional information in the final PDF. The summary can be viewed at any time in the task. At the end of the task, both employee and supervisor will receive an email to notify them that the task is complete.
